Ankle pain after running rarely has a single cause. It’s usually the result of a combination of factors: the surfaces you train on, the shoes you wear, your current fitness level, and how well your body has been prepared for the demands of running. Some of the most frequent general causes include:
- Uneven or unstable surfaces, such as trails, cobblestones, or cambered roads, which increase the risk of the ankle rolling or twisting.
- A sudden jump in training volume or intensity, which doesn’t give your tendons and bones enough time to adapt.
- Worn-out or unsuitable running shoes that fail to support or cushion the foot properly.

Peroneal Tendinitis

Symptoms: Pain along the outer ankle that intensifies during activity, swelling, warmth, and sometimes a thickened, nodular feeling along the tendon.

Sprains

Treatment: Most sprains respond well to the RICE method, with recovery times ranging from one to three weeks for mild sprains to two to three months for more severe ones. Following current evidence-based guidance, functional treatment — short-term protection followed by early, guided movement and rehabilitation exercises — tends to produce better long-term outcomes than prolonged immobilization.
Strains
Causes: Unlike sprains, which affect ligaments, an ankle strain involves the muscles or tendons around the joint. An ankle strain from running typically results from overstretching during a sudden movement, running while fatigued, or insufficient warm-up before a session.
Symptoms: Localized pain and tightness, muscle spasm, swelling, and difficulty moving the ankle through its normal range of motion.

Stress Fractures
Causes: An ankle stress fracture is a tiny crack in the bone caused by repetitive impact rather than a single traumatic event. Treatment: Stress fractures require a longer recovery than sprains or tendinitis, typically six to eight weeks of rest, activity modification, and sometimes immobilization in a boot. Sprained Ankle vs. Broken Ankle: What’s the Difference?
It isn’t always obvious whether you’re dealing with a sprained ankle or a broken one, since both can cause pain, swelling, and difficulty walking.
